The following information is required for the district’s report to the Arizona Department of Revenue to verify eligibility for the tax credit allowed by A.R.S. §43 - 1089.01. A receipt will be mailed to the donor.

- Select Program
- Complete Tax Credit Response Form
- Mail or Deliver Contribution to School Bookstore
Note: If a program is not designated, your contribution will be applied to the school’s general fund for extracurricular programs. Donations must be postmarked by April 15.
