2010-2011 On-Line Registration Forms
Casa Verde High School is proud to present registration forms online this year. We would like the process of registration to go smoothly so please use the checklist that is provided to help ensure you have all paperwork completed for registration. To make the registration process easier, we are asking that you complete all the information online and print. Please call our Guidance Office at 520-876-0661 and make an appointment to meet and set up your final schedule. Please read all information regarding the application before brining/sending it to 1362 N. Casa Grande Ave, Casa Grande, AZ 85122. You can also Fax in your paperwork to 520-876-0667.
This is a reminder that the application process for open enrollment starts NOW.
To Enroll at CVHS, you need the following:
- CVHS Course Booklet 2010-2011
- Immunization Records
- Transcript from previous high school OR 8th grade Promotion Certificate
- Birth Certificate
- Proof of Residency
- Picture ID with a parent OR Proof of Guardianship, if not parent

- Migrant Tutoring
- 504 Plans
